I do hereby fully release and discharge The Upper Room Dance Studio, it’s members, representitives, and/or employees from any and all claims from injuries, which my children or I might sustain. In the absence of my presence I authorize the responsible persons to administer and approve any necessary medical treatment for my child. I will assume all medical costs.
